Transportation Authorization - Chamblee Charter High School Swim & Dive Team
This is to certify that for the 2018-2019 school year, my child(ren) has permission to ride to and from an athletic practice or athletic contest in an automobile, which is driven by another person. I acknowledge that the DeKalb County School System assumes no responsibility for nor gives any assurances as to the safety or insurance coverage of the car or driver.
I also acknowledge that the Georgia High School Association (GHSA) assumes no responsibility for nor gives any assurances as to the safety or catastrophic insurance coverage of the car or driver.
Chamblee Charter High School Swim & Dive Team
This is to certify that my child(ren) has permission to drive his/her automobile, and to transport other persons, to and from athletic practices and/or athletic contests during the 2018-2019 school year.

I understand that attending practice means SIGNING
IN, being in the water on time, and staying for the duration of practice. If I forget to sign in, am late, or can not
stay for the duration of practice I understand that I need to make up that
practice…EVEN IF THAT MEANS ATTENDING PRACTICE AT A TIME THAT IS NOT
Swim Practice Requirements to remain on the team(Divers have an independent
practice requirement to earn a letter for Diving):
Minimum: 10 practices
Recommended: 15 practices
Encouraged: 18+ practices
Before Winter Break:
Minimum: 20 practices Recommended: 25 practices Encouraged: 26+ practices
Before MLK Weekend:
Minimum: 30 practices (will not letter/earn PE credit without 30
Club swimmers may continue to
train with their club teams and will provide the Chamblee coaches with their
club coach’s contact information to verify attendance.
Team information is sent via
email. Athletes are encouraged to check
their email daily but must check at least once per week. Attendance reports are sent weekly—any
discrepancies must be brought to the coaches’ attention within seven days of
the weekly attendance report being emailed.
Meet Attendance Requirements:
Swimmers and Divers must report for meets at the
designated check in time. ALL athletes are expected to attend
ALL meets (regardless of whether they compete) and are required to
stay until the end of the meets AND
must wear the official team swim cap and swimsuit to regular season
During the season, the
entire team is eligible to participate in nine meets. Athletes must attend a
minimum of seven meets to letter/earn P.E. credit (and, further, must not have ANY unexcused meet absences). Additionally, all
athletes are required to attend the DeKalb County Diving Championship
and/or the DeKalb County Swimming Championship to cheer on the team. All athletes who are selected for the
County Championship and State Championship teams are required to
participate in these meets.
To be excused from a meet or meet arrival
time means that a coach has given an athlete permission in advance to miss without
penalty. Excuses are granted in VERY limited
circumstances—such as a conflicting school function (EX:
CCHS-sanctioned sport, band concert, team banquet). Students need to communicate with the
coaches at least seven days in advance for the coaches to
determine whether the event will be excused or unexcused. Practices, meetings, tutorials, homework,
and rehearsals are NEVER considered excused.
Athletes are excused from
meets when they are ill and
absent from school if they provide the coaches with a doctor’s note or the
attendance office excused absence slip (you may send a picture of this to
the coaches via email) within 7 days of the absence. If
you are not at school for at least ½ a day you are not eligible to compete. If missing a meet please contact the
Coaches via email ASAP so that they can adjust the lineup.
I understand that the coaches
expect me to be a leader in the school, in the pool, and in the community. Therefore, I will abstain from illegal
substances in order to be a role model and to achieve the best performance in
the pool. Further, my behavior (both in
and out of school) should represent the Chamblee community in a positive
Students may be removed from
the team for conduct violating the Student Code of Conduct. Students who become academically ineligible
will be suspended from the team until they become academically eligible.
Consequences for failure to follow this Commitment
As a parent, I understand
that my swimmer/diver may not participate in every meet (and might not even
participate in any meets at all, due to the size of our team and the
limitations of entries for each meet). I
will encourage my son/daughter to attend practices and make them a priority
even when there is a heavy homework load.
I understand that should my son/daughter violate the above team rules
then they will be removed from the team.
All Swimmers and Divers must sign a hard copy of the Commitment Contract and submit it to Coach Reynolds or Coach Valley at their first practice.
