2025 Season Forms
All registrations, payment, and health forms must be completed before September 26th in order to be eligible to participate in practices.
Final Forms and Required Medical Forms
All athletes must complete all required forms in Final Forms by September 26th.
- Information about how to sign up in Final Forms is here.
- A complete physical examination signed by a doctor and dated after 4/30/25 is required to be uploaded. A copy of the required form can be found here:Athletic Consent and GHSA Physical Form (this form is updated 3/12/23) or in Spanish.
- You must also upload copies of Insurance Cards (front and back side) or apply for school insurance. (Please see Coach Reynolds in Room 1304 next to the gym if you need to apply for insurance).
You will not be allowed in the water until your file is marked complete. This may take up to 7 days to verify. DO NOT PROCRASTINATE OR YOU MAY BE INELIGIBLE TO SWIM/DIVE.
Commitment Contract
Parents will e-sign this contract when registering online. Please read through this contract in advance to ensure you and your swimmer/diver understand expectations.
The 2025-2026 contracts will be loaded online in September.

- Athletes will sign the Commitment Poster prior to the first meet
